Building Access Change Request Form
All requests for building access should be submitted through the departmental chairperson's office. This form is to be used for both departmental requests for building openings, and for individual schedule requests. The form must be submitted before any access or openings will be completed. All requests will be acknowledged by a reply email, so please verify your contact information.

We require at least 24 hour notice for all requests unless it is an emergency.
